URGENT DRUG RECALL – Sucralfate 1gm Tablets

On 7/14/2025, the drug manufacturer, Nostrum Laboratories, Inc, announced a consumer level recall for its Sucralfate 1gm tablets. If you received Sucralfate 1gm tablets from CenterWell Pharmacy, please contact us by telephone at 855-544-9419, Monday – Friday, 8 a.m. – 11 p.m. and Saturday, 8 a.m. – 6:30 p.m., Eastern time.


Product notice – Dexcom G6 and G7 Receiver

If you recently ordered a Dexcom G6 or G7 Receiver from CenterWell Pharmacy, we want to let you know that Dexcom, the manufacturer of the Dexcom G6 and G7 Receivers, has issued a voluntary recall of certain lots and serial numbers of these blood sugar monitoring medical devices (Dexcom Recall Notice). Dexcom stated the speakers of the G6 and G7 Receivers may fail and not make an alert sound when blood sugar is low or high.

This issue is isolated to certain lots of the Dexcom G6 and G7 Receivers (see product details below) and does not affect other Dexcom products or diabetic monitoring supplies you may have received from CenterWell Pharmacy.

What to do:

  1. Check the product list above to determine if your Dexcom G6 or G7 Receivers are recalled and affected. Dexcom’s website, www.dexcom.com/checkreceiver, can assist you.
  2. If your Dexcom G6 or G7 Receivers are recalled and affected, contact Dexcom directly at 1-844-478-1600 to receive a free replacement of Dexcom G6 or G7 Receivers. Dexcom is available 24 hours per day to assist you.
  3. Test your speaker regularly – Whether or not your Dexcom G6 or G7 Receivers are recalled and affected, test your receiver’s speaker every time you charge it. You can also re-test the sound at any time using the device menu. If the speaker does not beep when you test it, contact Dexcom right away. If the speaker beeps, be mindful that it may still fail without warning in the future.
  4. Make sure everyone who relies on the receiver for alerts (ex. patient, caregiver, etc.) knows about this possible problem.

 

If your Dexcom G6 or G7 Receivers are recalled and affected, continue to use your Dexcom products as directed by your healthcare provider, and immediately reach out to Dexcom for replacement product and further guidance.
