You can prescribe to one of our on-site retail locations by fax, phone or electronically. Use the physician fax forms above and send it directly to one of our retail pharmacy locations.
We work directly with healthcare professionals to make sure patients get the best possible service. We double-check each prescription, conduct medicine reviews and handle administrative tasks to ensure patients have what they need.
CenterWell Specialty Pharmacy® is in network with most Medicare plans and accepts non-Humana insurance. We offer 96% of specialty drugs in the market, including many hard-to-find, limited distribution drugs.

Prior authorization for pharmacy drugs
CoverMyMeds® is the quickest way to submit your prior authorization request. This is a free service that lets you submit and check the status of a prior authorization for any Humana plan. Registration is required.You can also find the appropriate form to mail or fax by going to Humana.com.
