Important information about your Vitamin D3 5000 IU Tablets

If you recently ordered the 21ST Century Vitamin D3 5000 IU Tablets through your Humana Medicare plan, we wanted to let you know the manufacturer has recalled select bottles.

The manufacturer of this product, 21ST Century HealthCare Co., Inc., issued a voluntary recall on May 19 for 1 lot code of the Vitamin D3 5000 IU Tablets because some bottles were mislabeled with a 1000 IU label, rather than a 5000 IU label.

The tablets inside the mislabeled bottles are 5000 IU Tablets. The mislabeled bottles could lead you to take incorrect dosages and may cause the following side effects— nausea, vomiting, muscle weakness, confusion, pain, loss of appetite, dehydration, excessive urination and thirst.

The manufacturer is not aware of any reports of illness or injury to members due to this recall. This recall is being conducted with the knowledge and approval of the Food and Drug Administration (FDA). Your health and safety are important to us. With that in mind, we need your assistance to determine if your bottle is a part of the recall.

List of affected items

The recalled lot code is listed below. To determine if your bottle is impacted, please check the lot code and label on your bottle.

Step 1: Check the lot code on your bottle. The lot code is located on the bottom of the bottle (see image below).

If the lot code on your bottle is 15769204, please go to Step 2 to determine if it’s impacted.

If the lot code on your bottle is different than 15769204, your bottle isn’t included in the recall and you can continue taking your vitamin tablets.

Step 2: If your bottle’s lot code is 15769204, check the front label to see if it says 1000 IU or 5000 IU.

If your bottle is labeled as 1000 IU (see image below), the bottle is impacted by this recall. Please contact CenterWell Pharmacy® immediately at 1-855-211-8370 (TTY: 711) to get a replacement of your vitamin tablets for free. We’ll also tell you how to get rid of the mislabeled bottle. We’re available Monday – Friday, 8 a.m. – 11 p.m., and Saturday 8 a.m. – 6:30 p.m., Eastern time.

If your bottle is labeled as 5000 IU (see image below), the bottle isn’t impacted by this recall and you can keep taking your vitamin tablets.
